We are seeking an experienced and compassionate Social Worker to join our multidisciplinary healthcare team on a permanent basis at Tokai Estate in the Southern Suburbs of Cape Town. Working within our rehabilitation facility and retirement village, you will provide essential psychosocial support to patients and their families throughout their care journey. You will work closely with an experienced multidisciplinary team to support patients with complex medical and psychosocial needs, including those receiving neurological, orthopaedic, surgical, palliative and respite care. This role is ideal for a Social Worker who is patient-focused, highly organised and passionate about supporting individuals and families through rehabilitation, adjustment and changes in care needs. Responsibilities include, but are not limited to:Conduct comprehensive psychosocial assessments and identify patients' social, emotional and practical needs. Provide counselling, emotional support and mediation to patients and their families. Contribute to discharge planning and help facilitate safe and sustainable transitions of care. Work collaboratively with the multidisciplinary team to coordinate patient care and rehabilitation goals. Identify and address psychosocial barriers that may affect recovery, treatment adherence or successful discharge. Facilitate referrals to appropriate healthcare, community and support services. Advocate for patients and their families throughout the care journey. Maintain accurate, professional and timely clinical documentation and reports. Participate actively in multidisciplinary meetings and patient discussions. Support patients and families navigating palliative care and complex healthcare circumstances. Assist with outpatient consultations where required. Requirements:Current SACSSP registration. Professional indemnity insurance. Minimum of 3 years' social work experience. Experience in hospital social work and case management. Training and experience in palliative care β essential. Strong counselling, interpersonal and communication skills. Strong report writing and time management skills. Ability to work effectively and collaboratively within a multidisciplinary healthcare team. Willingness to travel for outpatient consultations when required. Experience in geriatrics, oncology and curatorship will be advantageous.
